Sana’s vision is to make healthcare easy.

Healthcare is simply too hard in the US. At Sana, we're working to change that by making healthcare easier to access, navigate, and afford. We want our members to know exactly where to turn when they need care, with as few barriers as possible.

We all know navigating healthcare in the U.S. is confusing, costly, and frustrating — and our members are used to feeling that pain. That’s why we’re building something different: affordable health plans designed around Sana Care, our integrated care model connecting members with unlimited primary care and expert care navigation at no additional cost to them.

Whether it’s a quick prescription refill or guidance through a complex medical journey, Sana Care makes it feel effortless to get the right care at the right time. And for employers and brokers, we’ve built intuitive tools to make managing health benefits just as seamless.

If you love solving hard problems that make people’s lives easier, come build with us.

About the role

We're looking for a Contract Care Navigator to join our Sana Care team for a six-month assignment that has the potential to turn into something longer term. In this role, you'll help members navigate the healthcare system, connect with appropriate providers, and access the care they need.

You'll work closely with our experienced Care Navigation team, clinical providers and medical assistants, and other internal teams to coordinate referrals, answer member questions, and help resolve barriers to care. This is a hands-on, member-facing role that's ideal for someone who is organized, compassionate, detail-oriented, and excited to make healthcare easier to navigate.

What you will do::

  • Support Sana members in navigating the healthcare system and accessing appropriate, high-quality, cost-effective care.

  • Help members find providers, understand referral options, and coordinate access to healthcare services.

  • Communicate with members, healthcare providers, and external partners to gather information, resolve routine issues, and facilitate care.

  • Collaborate with Care Navigators, Sana Care providers, Medical Assistants, and Customer Support to ensure a seamless member experience.

  • Maintain accurate, timely documentation of member interactions and care navigation activities.

  • Follow established workflows, manage tasks through completion, and escalate complex clinical, benefits, or operational questions to appropriate team members.

  • Identify opportunities to improve workflows and share feedback with the team.

  • Help members feel heard, supported, and confident as they navigate their healthcare journey.

About you::

  • 2+ years of experience in a fast-paced environment, with at least 1 year directly in care navigation, referrals, provider scheduling, or healthcare benefits.

  • Strong written and verbal communication skills, with a warm, empathetic, and professional approach.

  • Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and follow through reliably.

  • Comfortable learning new technology, navigating multiple systems, and working in a fast-paced, remote environment.

  • Resourceful and proactive, with strong problem-solving skills and the judgment to know when to ask for help or escalate an issue.

  • A collaborative team player who welcomes feedback, communicates openly, and enjoys helping others succeed.

  • Adaptable and eager to learn, with a willingness to take on new tasks as team needs evolve.

  • Passionate about making healthcare more accessible, affordable, and easier to navigate for everyone.
